QDoc is an intelligent web-based system specifically built to address traffic management (queue) of healthcare centers such as clinics, children specialists, therapy centers etc. It is also a tool for business owners to keep track of their healthcare center performance based on queue entries.
Users of QDoc are categorized into two (2) categories, namely a) Healthcare center, and b) Patients. Hereinafter, collectively referred to as “Users”.
Healthcare center users are required to create an user account with QDOC GLOBAL- the service provider for the access of the system. And Healthcare center users are required to subscribe and pay the monthly subscription fee in order to keep their account active, UNLESS fee waiver is granted.
Whereas, patient users are not required to create an user account. However, personal information such as a) Name, b) Contact No, c) NRIC, d) Email etc need to be provided if patient users wish to “self-enter” queue number or make an appointment via QDoc. The reason for requesting the aforementioned information is for pre-registration purposes by Healthcare centers.
For walk-in patient users, QDoc only needs the user's name and contact number, because the “Queue Numbers” are generated by combination of patient’s first name and contact number, for instance [chong-011-1234xxxx].
This page explains the terms and conditions by which Users may use QDoc. By accessing or using the services of QDoc or by logging in, Users electronically represent (“sign”) that Users agree to the Terms and Conditions and further certify that Users have read, understood, and agreed to be legally bound to this “Terms and Conditions” (this “Agreement”).
We reserve the right to make changes to this Agreement at any time. If a change is material, we’ll let you know before it takes effect. By using QDoc on or after that effective date, Users agree to the new Agreement. If Users do not wish to be bound by such changes, Users may discontinue using and terminate the Service before the change becomes effective. Your continued use of the Service after the changes becomes effective, indicates your agreement to the changes.

Services
“Services” means all services described below and in
http://qdoc.cc, as updated from time to time. As long as Healthcare center users meet the payment obligations and comply with this Agreement, Users can access QDoc for the following key functions:-

Healthcare center users manage queues and appointments for their healthcare center via QDoc.

Healthcare center users keep track of the performance of their center.

Healthcare centers being listed in QDoc’s directory and search engine.

Patient users can self-enter queue numbers, make appointments, and check real-time queue status of healthcare centers which collaborate with QDoc.
Our Services does not include obtaining access to the Internet for connecting to the Services. You acknowledge that the operation and availability of the communications systems used for accessing and interacting with the Services can be unpredictable and may, from time to time, interfere with or prevent access to the Services or its operation. We are not in any way responsible for any such interference with or prevention of Users use of or access to the Services.
Also, in view of QDoc’s easy to understand interface, our Services do not include on-site training to Users. Tutorial materials and FAQ will be provided or posted in the QDoc online system in order to guide Users.
You agree not to:

use the Services other than as authorized in this Agreement;

use any device, software, or routine that interferes with any application, function, or use of the Services, or is intended to damage, detrimentally interfere with, surreptitiously intercept, or expropriate any system, data, or communication;

resell, sublicense, or otherwise share the Services with any third party;

frame or mirror the Services;

decompile, disassemble or reverse-engineer the underlying software or application that is part of the Services or otherwise attempt to derive its source code;

use the Services either directly or indirectly to support any activity that is illegal;

access the Services for purposes of monitoring its availability, performance or functionality, or for any other benchmarking or competitive purposes;

authorize any third party to do any of the above.
The Information We Collect
In the course of providing the Services, we collect personal information of Healthcare center users only for QDoc account creation, and Patient users information only for healthcare center pre-registration purpose.
The personal information may include but not limited to Users’ name, email, contact, NRIC, and address as well as the relevant information that would help us render the Services to Users.
We protect Users personal information by strictly complying with the Personal Data Protection Act 2010 (“PDPA”), the primary legislation concerning data protection in Malaysia. When using our Services, you consent to the collection, transfer, manipulation, storage, disclosure and other uses of your information only as described in this Agreement.
Non-Personally Identifiable Information. We automatically collect certain information when you visit our site - qdoc.cc and when you use our Services, such as Internet Protocol (IP) address, operating system, browser type, the address of the referring site, and any device identifier (if using a mobile device). We use this information to monitor and analyze the use of the Site and our Services, for technical administration of the Site, to increase the functionality and user-friendliness of our Site and our Services, and to better tailor it to our visitors’ needs.
Sharing And Disclosure Of Your Information
We also use Users information (and the information you provide to us) to perform and provide Users with the Services and to communicate with Users. We will not share personal information about Users with other people or non-affiliated companies, unless we have your permission, or under the following circumstances:

We may share any of the information we gather about Users with third party agents and service providers to facilitate the operation of the Site and Services, to provide the Site and our Services or functions on our behalf and to perform related services (e.g., without limitation, maintenance services, database management, web analytics and improvement of the Service’s features), or to assist us in analyzing how our Site and Services is used. These third parties may have access to personal information needed to perform their functions, but may not use it for other purposes. These third parties may be located outside Malaysia.

We will cooperate with the Malaysian government and polis and private parties to enforce and comply with the law and will disclose any information about Users to government, polis or private parties as we, in our sole discretion, believe necessary or appropriate to respond to claims and legal process, to protect our property and rights or those of a third party, to protect the safety of the public or any person, or to prevent or stop any activity we may consider to be, or to pose a risk of being, illegal, unethical or legally actionable.

We may share aggregated information that does not include personal information and we may otherwise disclose such information with third parties for industry analysis, demographic and analytical profiling and other purposes. Any aggregated information shared in these contexts will not contain your personal information.

We may sell, transfer or otherwise share some or all of our assets, which could include your personal information, in connection with a merger, acquisition, reorganization or sale of assets or in the event of company liquidation.
Changing Your Information
Users may update, or correct their personal information by contacting us. If Users terminate the Services, then Users account will become deactivated and Users will not be able to access our Services. If you would like us to delete your record in our system, please contact us with a request that we delete your personal information from our database. We will use commercially reasonable efforts to honor your request, however, we may retain an archived copy of your records as required by Malaysian law or for other legitimate business purposes.
Security
We will take reasonable efforts and actions to protect your information from unauthorized access, however, despite those efforts, no security measures are perfect or impenetrable and no method of data transmission can be guaranteed against any interception or other type of misuse.
Liability
QDoc is an online system that manages queue sequence of queue numbers entered by Users. It is the responsibility of the Users to use and enter accurate information to QDoc in order for the system to work effectively. We will not be liable for any liability that results from inaccurate or incomplete information Users supply.
Termination
If you violate the letter or spirit of this Agreement, or otherwise create risk or possible legal exposure for us, we reserve the right to stop providing all or part of our Services to you. We will notify you by email or at the next time you attempt to access your account.
